What to Expect
Prior to your first appointment, you will need to complete your patient data sheet. This information is VERY helpful in providing the doctor(s) with a broad overview of your health condition(s), as well as your family’s health history, personal diet, exercise habits, and what patient care you have received in the past. It also gives us basic contact information, as well as billing information if you would like us to file insurance for you. This data sheet may be downloaded directly from our website at
www.musldoc.com/patientdatasheet. If you are unable to complete the patient data sheet prior to your appointment, we ask that you arrive 30 minutes early to allow adequate time to fully complete this information. If your form is fully completed, we ask that you arrive 10 minutes early.
After reviewing your patient data sheet, your vital signs such as height, weight and blood pressure will be assessed. Your doctor will then perform a thorough orthopedic and neurological examination including posture analysis, and a detailed muscle performance evaluation related to your concern. These services will be performed exclusively by your doctor. Any recent imaging or labs such as x-ray, MRI or blood work relating to your concern you bring in will be helpful and reviewed as part of your visit.
At the end of the examination, your doctor will give a complete report of his findings, answering 3 important questions:
1. What is wrong
2. Can we help, and
3. How long should it take.
Of course your doctor will answer any additional questions you might have, as well as explanation of therapies. In some circumstances imaging and or additional testing will be required and will be referred to the appropriate lab or imaging center. In most cases you can be treated that day if you so choose, as time allows. If you have any questions as your treatment progresses, please ask. Our doctors love to answer your questions!

How Do I Pay for It?
Health Insurance – We do everything necessary to help you get reimbursement from your health insurance for services in our office.
If you have Blue Cross Blue Shield, we are contracting providers, and we make every effort to accurately estimate your coverage based on your plan. We ask that you pay your expected portion in full at the time services are rendered.
If you have a different health insurance company, we will be happy to provide you all documentation needed to self file to receive reimbursement directly from your insurance company. However, we require that you pay in full at the time of service on each visit.

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
